What is Vaginoplasty?

Pelvic relaxation is the term used to describe changes in the pelvic floor muscles and fascia that such as weakening of these tissues so that the pelvic organs may prolapse in time due to the effect of gravity.

There are several reasons for pelvic relaxation:

• Childbirth-especially with vaginal delivery of macrosomic (overweight) infants may increase the risk, however even with C-Section, there is still a risk of pelvic relaxation. Pregnancy itself causes increased intraabdominal pressure that weakens the pelvic tissues.
• Aging-aging may decrease tissue strength especially if the patient is not exercising and is overweight.
• Menopause: Decreased estrogen levels in post-menopausal patients causes atrophic changes in the vagina and pelvic floor that lead to pelvic relaxation
• Chronic increased intraabdominal pressure due to constipation, chronic coughing and obesity may weaken pelvic floor muscles
• Heavy lifting at work or during exercise
• Genetic factors-Some patients have weaker tissues compared to other patients of the same age due to genetic reasons. In such patients, pelvic relaxation may start at younger ages.

What changes that occur in the vagina necessitate a vaginoplasty procedure?

Weakening of pelvic tissues may lead to a larger vaginal opening and canal and even some tissues may protrude outside from the vaginal opening.

Depending on the degree of pelvic relaxation different symptoms may occur. In mild forms of pelvic relaxation the vaginal muscles may be week, sexual life may be unsatisfactory, there may be involuntary loss of urine or gas. In severe pelvic relaxation the pelvic organs such as the uterus, bladder and rectum may be visible totally outside.

In addition vaginal birth may cause tears in the vagina and lead to badly healed episiotomy scars.

Is there a non surgical alternative to vaginoplasty?

Kegel exercises are designed to strenghten pelvic floor muscles. When they are repeated regularly, they may have an effect especially in mild forms of pelvic relaxation.

Laser treatment has also been designed for patients who do not prefer to have surgery. However non of these methods are as effective as a vaginoplasty procedure.

What is vaginoplasty?

Vaginoplasty is also known as vaginal rejuvenation. It is the surgical procedure performed to tighten the vaginal enterance and the vaginal canal. Excess vaginal mucosal tissue is removed and the underlying levator ani muscles are plicated to tighten the vaginal canal.

If necessary, concommitant procedure of perineoplasty may be performed on the perineum (the part between vaginal orifice and anus). Badly healed episiotomy scars and hyperpgmented tissues may be removed for an esthetic apperance on the vulva. This procedure is more effective than other vulvar bleaching methods.

Also labioplasty, clitoral hood reduction, surgeries to the mons pubis and labia majora may be performed during a vaginoplasty procedure.

Am I a Candidate for Vaginoplasty?

They should be in generally good health condition and should wait for about 12 months after childbirth before surgery.

What type of anesthesia is necessary for a vaginoplasty procedure?

Vaginoplasty surgery is performed either under local anesthesia with sedation or general anesthesia. Dr. Saygan- Karamürsel will determine the best type of anesthesia based on your procedure and examination. Anesthesiologists in our team with experience and expertise will be attending your surgery.

How long does the vaginoplasty procedure last ?

The procedure will take 1-2 hours and all scarring with vaginoplasty is internal. If perineoplasty is performed, there may be esthetic sutures on the vulva that dissolve by themselves. Most of the time, vaginoplasty is performed as an outpatient procedure. After having some rest in the hospital or clinic, the patient can go home on the same day.

The Recovery Process

You can usually go home a short time after your procedure, but resting and relaxation is important. A control appointment will be scheduled after surgery.

You will see Dr. Saygan-Karamürsel the day after your surgery and at regular visits after surgery. This is done so the doctor can personally monitor your recovery process and to ensure that you have the best results possible.

Following your surgery there will be some bruising, swelling and pain. You will have a vaginal pack, which includes a sterile gauze. It will be removed the morning after your surgery by your doctor

Your discomfort will be controlled by the pain medication prescribed by Dr. Saygan-Karamürsel. To help with the swelling, you should apply ice packs for the first 48 hours.

You will go home walking after your procedure and you may need to take one week off from work following your surgery. You may be very sensitive over the next 2-3 weeks and you should avoid any strenuous physical activities.

You should avoid any high impact activities like jogging for up to six weeks. Full recovery takes from 4-6 weeks and you can resume sexual activity after 6 weeks.

You should not swim or wash in the tub for 6 weeks, but you can take a shower even on the day of surgery. It is important to keep the wound dry and clean.
